Who can use the Integrated Care SEP?

The Integrated Care SEP lets a full-benefit dually eligible individual (QMB+, SLMB+, or FBDE level Medicaid) enroll in an integrated D-SNP in any month, to align their Medicare and Medicaid coverage with the same organization.

Who qualifies

Full-benefit dually eligible individuals — QMB+, SLMB+, or FBDE level Medicaid. Partial-benefit duals and LIS-only individuals use the separate Dual/LIS SEP instead.

What it's for

Aligning enrollment with an integrated D-SNP (FIDE, HIDE, or AIP) and its companion Medicaid managed care organization, so both plans are coordinated under the same parent organization.

"Other SEP allowed"

Non-AIP integrated D-SNPs can only use this SEP to align enrollment to an affiliated Medicaid MCO. AIP D-SNPs generally have broader alignment flexibility — always verify current CMS guidance for the specific plan.

FIDE / HIDE / CO / AIP — quick legend

FIDE SNPFully Integrated Dual Eligible SNP — one entity holds both the Medicare Advantage contract and a Medicaid managed care contract covering the D-SNP's full service area, including LTSS and behavioral health.
HIDE SNPHighly Integrated Dual Eligible SNP — Medicaid benefits (LTSS, behavioral health, or both) are covered under a capitated Medicaid managed care contract, through the D-SNP itself or an affiliated plan.
CO D-SNPCoordination-Only D-SNP — no affiliated Medicaid managed care plan and no state capitation for Medicaid benefits; the D-SNP coordinates delivery of services but doesn't cover them directly.
AIPApplicable Integrated Plan — a FIDE SNP, or a HIDE/CO D-SNP with exclusively aligned enrollment plus expanded Medicaid coverage; AIPs get unified Medicare/Medicaid appeals and grievance procedures.
